What the inspector wrote

On June 3, 2026, Licensing Program Analyst (LPA) Van arrived at the family child care home to conduct an unannounced Plan of Correction (POC) inspection. LPA met with the licensee, Zhanneta Karapetyan, explained the purpose of the visit, and was granted access to the home. Upon entry, the licensee and an assistant were supervising 6 children in care (5 preschool-aged and 1 infant). The licensee operates in compliance with her license capacity and limitations. During the annual inspection conducted on May 5, 2026, LPA observed violations related to off-limits areas and emergency disaster drill requirements. LPA observed an assistant changing a child’s diaper in a bedroom designated as off-limits and not approved for child care use. LPA also determined that the licensee had not conducted and documented emergency disaster drills at least once every six months; the last documented drills were completed in January 2025. Additional technical violations were issued for failing to maintain required personnel immunization documentation, for stairs accessible to children under five not being properly barricaded as required for home safety, and for failing to ensure that parents or authorized representatives signed and dated the LIC 995A acknowledging receipt of the Parents’ Rights Notice. During today’s visit, LPA verified through observation and records review that all previously cited deficiencies, including the technical violations, have been corrected. The licensee ensured that all off-limits areas were made inaccessible, and LPA confirmed that the previously used bedroom was secured. The licensee also conducted and documented current emergency disaster drills, and updated drill logs are now maintained in the home. LPA further verified through records review that the technical violations related to personnel immunization documentation, barricading of stairs for children under five, and parent signatures on the LIC 995A have also been corrected. Based on today’s inspection, all deficiencies cited on May 5, 2026, have been corrected. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with the licensee, Zhanneta Karapetyan.
