What the inspector wrote

On November 18, 2025 at approximately 2:30PM, Licensing Program Analyst (LPA) Katie Krenn conducted an unannounced case management visit for an incident that the facility self reported. LPA met with the facility representative, Mayra Monje and supervisor, Rondell Howard explained the purpose of the visit. In addition to the facility representative and supervisor there are seven staff supervising 43 school age children. All staff present during today's visit have been finger print cleared through the Department of Social Services. One staff member is not associated with this location. That staff is associated with a different location of the same program. LPA interviewed, facility representative, Mayra Monje and supervisor, Rondell Howard together regarding the reported incident. Facility representative and supervisor explained their response to the incident which included speaking with the children involved, notifying the parents, increasing visual supervision in the outdoor area, and a scheduled training for staff. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. continued on page 2 Page 2 Incidental Medical Services (IMS) policy was discussed.
