What the inspector wrote

On February 26, 2025 at approximately 3:30PM, Licensing Program Analyst (LPA) Katie Krenn conducted an unannounced annual inspection visit. LPA met with the facility representative, Mayra Monje (R1) and explained the purpose of the visit. In addition to the facility representative there are six staff supervising 39 school age children (kindergarten - fifth grade). All staff present during today's visit have been finger print cleared through the Department of Social Services. The facility operates in rooms 108 and 143 as well as an outdoor play area. Days and hours of operation during the school year are Monday through Friday from noon to 6:00PM. Days and hours of operation during school breaks are Monday through Friday from 8:00AM to 6:00PM. LPA observed that all required postings, including the facility license, Notification of Parents\342\200\231 Rights, Personal Rights, Car Seat Law, and Emergency Disaster Plan, were displayed and visible in the hallway area where children\342\200\231s personal belongings are stored. LPA did a physical plant inspection and did not observe any deficiencies. LPA also discussed a capacity increase application with R1. LPA observed the classrooms to be clean, equipped with age-appropriate furniture and educational materials. Flooring in classroom is tile which had minimal scuff marks and small debris. LPA observed storage for children\342\200\231s belongings in the hallway, labeled for each child. continued on page 2 Page 2 To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. No deficiencies were cited during today's inspection. This inspection will be continued at a later date. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the facility representative, Mayra Monje.
