What the inspector wrote

On 7/7/2025, at approximately 9:30 AM, Licensing Program Analysts (LPAs) Wadhwa and Olguin-Leon conducted an unannounced, annual inspection. LPAs met with the Parish Administrator, Chantal Eshghipour. Director, Cara O\342\200\231Connell arrived at the facility about 30 minutes later and was explained the purpose of the visit. Both adults present today have fingerprints and background clearance on file. The facility operates a preschool age program in Room#1 \342\200\234Butterflies\342\200\235 (2-3 year olds) and Room#2 \342\200\234Honeybees\342\200\235 (3-4 year olds). The facility has an outdoor area divided into three parts: a playground with an age appropriate play structure, an upper-level play area with a sand box and vegetable garden, and a patio that is used for picnics. Hours of operation are Monday through Friday 8:30am to 4:00pm. There were no children or staff present at today\342\200\231s visit as the program is not operating today. The last day of school\342\200\231s summer program was 7/3/2025. The next school year program will begin on 8/27/2025. LPAs toured the facility with the Director to conduct a health and safety inspection. LPAs observed classrooms to include a variety of toys and materials that were in good condition. The classrooms have age-appropriate furniture, equipment, and materials. Storage for children's belongings is inside the classroom. LPAs observed cleaning products to be stored under closed cabinets under bathroom sinks. LPAs reminded the Director to put locks on the cabinets to ensure inaccessibility to children while in care. Per director, the program was being run from 8:30 AM to 1:00 PM in the last school year, however, the program will be operating from 8:30 AM to 2:00 PM this school year. The Director noted that a nap/rest time will be incorporated in the next school year schedule. Continued on Page 2 Page 2 Classrooms are equipped with a fully charged fire extinguisher and first aid kit. LPAs observed Carbon monoxide detectors and smoke detectors in the classrooms. LPAs were unable to test the carbon monoxide/smoke detector due to being placed high up in the classrooms. Per Director, there will be new carbon monoxide/smoke detectors, and an emergency fire-alarm pull- station installed before the beginning of the new school year. There is one bathroom inside each classroom and one bathroom outside of the classroom available for the children to use. Each bathroom has one sink and was observed to be in working condition. Per Director, the children bring lunch and snacks from home. Parents provide a water bottle from home and the facility provides refill as needed using bottled water. Outdoor areas are entirely enclosed and fenced. LPAs observed outdoor play areas to have roof, for shade. LPAs observed a play structure to have cushion flooring. LPAs observed a sandbox to be well maintained. LPAs observed toys and material to be in good condition. Program uses \342\200\234Brightwheel\342\200\235 app for parents to sign in and out and a paper sign in - sign out sheets in each classroom. LPAs reviewed sign in/sign out sheet for 7/3/2025 and observed three out of twelve children were not signed out properly. Advisory Note: Technical Violation (LIC9102TV) was issued. LPAs observed licensing posters and required documents to be properly posted. The program\342\200\231s last disaster drills were conducted on 5/13/2025. LPAs reviewed children\342\200\231s facility records which were complete and had all required documents. Children's records have emergency information on file. LPAs reviewed facility staff records and found some staff to be missing immunizations and other required documents available for review today. Advisory Note: Technical Violation (LIC9102TV) was issued. Continued on Page 3 Page 3 Director was reminded all staff hired are required to complete Mandated Reporter Training every two years. For additional information regarding the inspection and its tools and methods, please visit the Program website at www.cdss.ca.gov/inforesources/community-care-licensing/inspection-process Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. Continued on Page 4 Page 5 LPA's discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Please see LIC 809-D for deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. LPAs provided director with Appeal Rights. Exit interview conducted and report was reviewed with the director, Cara O\342\200\231Connell.
