What the inspector wrote

On October 17th, 2024, at approximately 2:15 PM, Licensing Program Analyst (LPA) Janet Gil conducted an unannounced, plan of correction (POC) visit. LPA met with licensee, Leila L. Anico, and explained the purpose of the visit. Present during today's visit included licensee, spouse, and 6 enrolled children (6 Preschoolers). On October 10th, 2025, at approximately 1:40 PM, LPA Gil conducted an unannounced, annual inspection. During annual inspection, licensee was issued one Type B citation for missing Infant sleep logs. As of this date, LPA observed licensee to have an infant sleep log for the one enrolled infant child in her Family Child Care Home Day Care. LPA observed licensee also created her own version of the infant sleep log for personal use. During visit, LPA also observed Notice of Site Visit to be properly posted and available for review. LPA confirmed licensee understands the importance of a sleep log, requirements to have one for each enrolled infant, and importance of properly filling out the time slots every 15 minutes. Deficiencies cited on October 10th, 2025, have been cleared during today's visit. LPA provided licensee with a POC letter. No deficiencies were cited today . A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with licensee, Leila L. Anico.
