What the inspector wrote

On 04/02/25 at approximately 2:30pm Licensing Program Analyst (LPA) Katie Krenn conducted an unannounced annual inspection. LPA met with licensee, Isabel Pasallo and explained the purpose of today\342\200\231s visit. Present in the facility are licensee and an assistant caring for six children (4 infants and 2 preschool age). Licensee is operating within capacity limits. The licensee's adult child recently moved in and is not fingerprinted. Per licensee, they did not know that the adult living in the main home needed to be fingerprinted since they are not involved with the day care building, which is a separate building at the same address. Licensee owns home, which is a 3 bedroom, 1 full Bathroom, 2 half bathrooms, and an In-Law unit in the backyard with a full bathroom. Licensee lives with spouse and adult child. The hours of operation are Monday - Friday from 7am - 6pm. Daycare areas: Backyard and Entire In-law unit with bathroom. OFF limit area: Entire Main house: Living Room, Dining Room, Family Room, Garage, Bathroom #1, half bathrooms #1 and #2, Bedroom #1, #2, and #3. All off limit areas, including closets, are properly barricaded. LPA observed that required documents, such as the facility license was displayed and visible to the public. However, the Notification of Parental Rights and the Earthquake Preparedness Checklist were not posted. LPA printed copies of both so that licensee could post them immediately. Continued on page 2 Page 2 The most recent emergency disaster drill was conducted on 2/24/25, and the LPA observed that it was documented. LPA reviewed the emergency disaster drill log and found they were conducted at least once every six months. LPA inspected the indoor and outdoor daycare areas to identify health and safety hazards. The home had a fire extinguisher, a first aid kit, and multiple carbon monoxide and smoke detectors. LPA did not test the smoke and carbon monoxide detector because the children were napping. LPA observed that the home's interior was clean and orderly, with proper heating and ventilation. Disinfectants, cleaning solutions, poisons, and other hazardous items were stored in areas inaccessible to children. The bathroom was clean, fully operational, and equipped with suitable toileting equipment and sanitation products. LPA observed that the daycare areas had age-appropriate toys, furniture, and educational materials. There is no fireplace. A designated area was provided for permanent or portable storage of the children's clothing and personal belongings. Electrical outlets were fitted with child safety covers to prevent children's access. According to the licensee, there are no firearms or weapons on the property. The backyard is surrounded by a fence and features a variety of outdoor play equipment, all of which are in excellent condition. The areas around and under high climbing equipment, swings, slides, and similar equipment were observed to be cushioned with artificial grass. LPA did not observe any pools, spas, or bodies of water on the property. The licensee stated she currently does not provide lunch for the children, lunches are provided from home in lunch bags that the children bring everyday. LPA reviewed the licensee\342\200\231s files. Files for licensee and staff were not available. LPA when over the forms that should be kept on file and printed LIC 9108 for licensee and staff to complete. LPA confirmed that the licensee holds a valid Pediatric First Aid/CPR certification, which is set to expire in September 2026. LPA reminded the licensee to maintain immunization records in the staff files. Continued on page 3 Page 3 The facility offers different options for children to rest or nap, including cribs and sleeping mat. Each child has their own crib or mat. Linens are washed weekly. LPA observed that sheets and blankets were available for the children. Infant mattresses are firm and equipped with appropriately sized sheets. All cribs are well-maintained and free of loose items that could pose safety risks . LPA reviewed the files of six children and confirmed that all necessary forms were present and completed for each child. The files were complete. The 15-minute sleep records were not available for review. LPA reviewed Safe Sleeping Guidelines. LPA printed a LIC 9227 and 4 sheets for 15-minute checks. LPA reminded the licensee to maintain form LIC 9227 for children under one in their file s. During the inspection, LPA observed a baby walker in the closet. Per licensee, this was provided by a parent and has not been used recently. LPA observed the parent removing the baby walker when they came to pick up the infant. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/ safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Continued on Page 4 Page 4 LPA issued two type B citations. LPA Katie Krenn informed licensee that this report dated 04/02/25 document one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Katie Krenn informed the licensee to provide a copy of this licensing report dated 04/02/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights were given to the licensee and LPA reviewed them. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee Isabel Pasallo.
