What the inspector wrote

On October 6, 2025 at approximately 9:15AM Licensing Program Analyst, (LPA) Katie Krenn arrived at the facility for a case management visit of an incident that the facility self reported. LPA met with Center Director, Grace Corpuz and explained the purpose of today's visit. Present at the facility in addition to the director were seven staff supervising 29 preschool children. The facility is operating within capacity limits and ratio during today's visit. Hours of operation are Monday through Friday 7:30am to 6:00pm. Preschool is licensed to operate in nine classrooms: Room 103, 110A, 110B, 112, 150 (re-labeled to #15), 157 (re-labeled to #23),160 (re-labeled to #17A and #17B), 166 (re-labeled to #20), and 168 (re-labeled to #18A and #18B) . Preschool program is currently operating in three classrooms: Farm class #23 (2 year olds), Ocean class #18A (3-4 year olds), Rainforest class #18B (4-5 year olds). Preschool program is also currently using Sensory Room #20 and Therapy Room #21. The facility reported that during a recent safety drill on 09/19/25, one child did not transition from the playground to the classroom with the rest of their classmates. When the staff realized that the child was missing, one staff went to the playground to look for the child. The child was found right away. The child was returned to their classroom unharmed. Per the interviews and the facilities self report, the child was unsupervised for approximately seven minutes. continued on page 2 Page 2 The facility notified the parents, did their own investigation and self-reported to licensing as required. Prior to today's visit the facility had a training on attendance procedures using Teaching Strategies with all staff. Additional staff have been added during transitions to provide support, reinforce transition procedures, and the importance of using the name-to-face method during every transition. During today's visit the LPA inspected the classroom and playground, including the transition areas. LPA also interviewed three staff members. LPA did not observe any health or safety hazards during the inspection. To improve the quality and value of the inspection process, a survey will be sent to the email address provided. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted, appeal rights were reviewed, and report was reviewed with the director, Grace Corpuz.

Supervision shall include visual observation.This requirement is not met as evidenced by: Based on the facility's self report, this requirement was not met as a child was left unsupervised for approximately 7 minutes. This poses an immediate health and safety risk to children in care.
