What the inspector wrote

On 9/22/2025, at 9:17 AM, Licensing Program Analysts (LPAs) Wadhwa and Leong conducted an unannounced case management visit to deliver an amended copy of the annual inspection report dated 9/8/2025. LPAs met with Director, Shannan Gotschall, and explained the purpose of the visit. Present during the visit were 13 children and 6 staff members. All staff present were verified to have fingerprint clearance. During the 9/8/2025 annual inspection, the facility was erroneously cited for having a staff member without a criminal record clearance. Upon further investigation, it was determined that the fingerprint clearance was available, and the error occurred due to a clerical issue within the Guardian system. The facility was notified via email on 9/10/2025 that the citation would be rescinded and the annual report amended. At today\342\200\231s visit, LPAs reviewed the amended report with the Director and provided a corrected copy. The facility also received an amended copy of the Notice of Site Visit dated 9/8/2025. A copy of this report was reviewed with and provided to the Director. The Director\342\200\231s signature on this report confirms receipt. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, Shannan Gotschall.
