Correction check·April 17, 2025

On 4/17/2025, at approximately 3:20PM, Licensing Program Analyst (LPA) Alvarado conducted an unannounced Plan of Correction (POC) visit at the facility. LPA met with License Maria Martinez (L1) explained the purpose of the visit. Present during the visit was Licensee, and Two Assistants supervising a total of 3 (Infants), 4 (Preschool Age) and 3(School Age Children). Facility is licensed for a large license. At 3:25PM one child was picked up. During the Plan of Correction visit on 3/18/2025, the Licensee received a citation regarding Infant Safe Sleep. At 3:25 LPA Alvarado along with (L1) inspected the home for any health or safety hazards. Prior citations were not observed again. Safe sleep was discussed and reviewed again as a reminder. As of today 4/17/2025, the citation issued on 3/18/2025 will be cleared. Deficiencies cited on 3/18/2025, were cleared today. The Letter of Deficiency Citation Cleared were provided to the Licensee. At approximately 3:45PM and 3:55PM, Three more children were picked up. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee, Maria Martinez.
