What the inspector wrote
On 12/10/2025 at 2:25PM., Licensing Program Analyst (LPA), Luis Gomez met with Licensees, Jack Lee and Wendy Mai. The purpose of today\342\200\231s visit was explained and was for an unannounced, annual/ random inspection. Present were licensees caring for 4 children. ( 1 preschool-age, 3 infant-age) Adults have criminal record clearance on file. The home is a four-bedroom, two-bathroom, two level house. The areas of the home designed for childcare include: (Lower Level) Bedroom # 1, the Sun Room, the Living Room, the Dining Room, and Backyard. The areas of the home designated as off-limits: (Lower Level) Garage, Kitchen, and Foyer. (Upper Level) Bedroom #2, #3, #4, and Bathroom #2. Per licensee, days and hours of operations are: Monday- Friday, 8:00 AM., to 5:00 PM. LPA inspected home inside and outside for health and safety hazards.
At 2:30PM., the following was observed: Facility clean and neat, with age-appropriate playthings available for the children. The floor and ground surfaces were clear of obstructions or hazards. The furniture, materials, and toys were in good repair, and free of sharp corners. LPA observed infant feeding chairs, with detachable table in kitchen. Accessible staircase has been barricaded.
For napping services/ scheduled rest, LPA observed several play pens in bedroom #1. Per licensee, sheet and bedding are washed weekly.
The facility bathroom (1) was observed clean, with faucet and toilet in operating condition. The facility was comfortable with ventilation and sufficient lighting. The facility was equipped with a telephone service; functioning carbon monoxide/ smoke combination detectors; and fire extinguisher (2A:10BC) located in kitchen. (REFER TO 809C, FOR CONT.)

LPA inspected the outdoor play area/ patio. The area was completed enclosed with playthings in good repair. LPA observed padding surface has been installed for added safety. The patio area was free of thorned or hazardous plants. The home does not have any pools; fishponds; jacuzzi; or other bodies of water.
LPA reviewed facility records including the children\342\200\231s files. The children\342\200\231s files were reviewed and included the: Identification and Emergency Information (LIC700); Consent for Medical Treatment (LIC627); Notice of Parents Rights (LIC995A); and Notification of Parent's Right (LIC995A).
The licensee is documenting infant napping conditions during each 15-minute review. LPA reviewed napping log during inspection.
LPA confirmed both licensee\342\200\231s cardiopulmonary resuscitation (CPR)/ Pediatric First Aid Certifications were current, expiring on 4/2027. LPA confirmed both the licensee\342\200\231s proof of the completed mandated reporter training course (AB1207) was current, expiring 9/2026.
The licensee conducts the required emergency disaster drill every 6 months, with last drill done on 9/8/2025, and is properly logged.
The required forms are posted in facility and include the: Facility License; Notification of Parent\342\200\231s Rights (PUB379); Earthquake Preparedness Checklist; and the Emergency Disaster Plan (LIC610A).
Per licensee, isolation of an ill child is in the playroom. Per licensee, the facility provides daily snack/ meals service for children. (REFER TO 809C, FOR CONT.)

The licensees were reminded that all adults 18 years and over living in the home, person who provides care and supervision to children, and staff who have contact with children, including employee and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain criminal clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

as an additional resource. LPA informed licensee of the importance of checking for recalled infant devices on United States consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

site is a consumer education website that helps families obtain child care by connecting to child care providers and resources and referral agencies (R&R) throughout California.

Based on today's inspection, no deficiencies were cited in areas evaluated according to California Title 22, Div. 12 Chap. 3, Health and Safety Code of Regulations. The exit interview, including the facility evaluation report was discussed with Licensee, Jack Lee and Wendy Mai. Their signature on this form acknowledges receipt of these documents.
During exit interview, licensee confirmed no registered sex offenders are living in the home, and RSO profile was completed by LPA. The notice of site visit must remain posted for 30 days.
