What the inspector wrote

On October 7th, 2025, at approximately 10:00 AM, Licensing Program Analyst (LPA), Janet Gil, conducted an unannounced case management visit for PFS Bayshore Midway Street CDC's preschool license. The purpose of the visit was to follow up with a unusual, reported incident involving a child(C1) taken into hospital after day care hours on September 25th, 2025, for being "not responsive" and having "glazed eyes". LPA met with director, Teri Shepard, and explained the purpose of the visit. Present during inspection for the preschool license included 55 enrolled preschool children, and 13 staff (including director). Facility operates as a dual license. Present for the infant license included 15 infants and 5 staff. All staff present at the facility providing care have fingerprint clearance on file. Preschool program currently operates Monday through Friday 7:00 AM to 5:45 PM. Facility is operating within its capacity and in accordance with the required ratio of staff to children. Director stated C1 has been dis-enrolled and did not return to the facility after the incident. Per director, facility was aware of incident in detail due to a statement from the police department. No deficiencies were issued today during LPA's visit. A copy of today’s report was given to the lead teacher, Teri Shepard. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the director, Teri Shepard.
