What the inspector wrote

On May 12th, 2025 at approximately 2:45 PM, Licensing Program Analyst (LPA), Janet Gil, conducted an unannounced case management visit for PFS BayShore Mission Street CDC's infant license. The purpose of the visit was to follow up with a unusual reported incident involving a child(C1) who fell and cut his upper eyebrow on 5/1/2025. LPA met with director, Kelley Wheatley, and explained the purpose of the visit. All staff present at the facility providing care have fingerprint clearance on file. Present during inspection for the infant license included 11 enrolled children, and 6 staff (including director). Preschool program currently operates Monday through Friday 7:00 AM to 5:45 PM. Facility is operating within its capacity and in accordance with the required ratio of staff to children. Director stated C1 returned to school on 5/6/2025, and is doing well, and is active. Per director, parent let facility know C1 was taken to the doctor's and received glue to close wound/cut. C1 has returned to facility care, but was not present on today's visit. No deficiencies were issued today during LPA's visit. A copy of today’s report was given to the lead teacher, Kelley Wheatley. A Notice of Site Visit was given and must remain posted f or 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the director, Kelley Wheatley.
