What Happened
Case Management visit investigating personal rights violation. On October 15, 2024, a staff member and child simultaneously used adjacent restroom toilets with no privacy barriers. Staff member was terminated October 16, 2024. Type 22 Division 12 Chapter 1 citation issued.
Observations
- PERSONAL RIGHTS VIOLATION: Child (C1) and staff member (S3) used children's restroom simultaneously with no privacy between toilets
- Four toilets in children's restroom aligned next to each other with no privacy barriers between them
- Half-partition doors present but inadequate privacy

LPA observed children's restroom to have no privacy between children's toilets. This posed a potential health, safety or personal rights risk to children in care.
