What the inspector wrote

On May 19th, 2026 at approximately 9:10AM, Licensing Program Analysts (LPA) Alvarado conducted an unannounced plan of correction (POC) visit. LPA met with licensee, Ana Bertha Santiago(L1), and explained the purpose of the visit. Present during visit included (L1) supervising 4 children (3Infants and 1 Preschooler). The Facility is a large license and is operating within capacity limits and ratio during today’s inspection. (L1) was reminded that if no assistant provider is present at a Large Family Child Care Home, then the licensee shall comply with the capacity requirements for a Small Family Child Care Home as specified in 102416.5 Staffing Ratio and Capacity. All adults present during today’s inspection have criminal record clearance and are associated to the facility. On May 5, 2026, LPA Alvarado conducted an unannounced annual inspection. During the annual inspection, LPA issued 1 Type A citation in regard to Staffing Ratio & Capacity, regarding facility being overcapacity where LPA observed (L1) supervising 4 infants and 1 Preschooler. LPA also issued 2 Type B citations in regard to Infant Safe Sleep and Immunization's. A plan of correction was discussed with licensee. LPA Alvarado along with (L1) conducted a Health and Safety Walk through of the facility indoors and outdoors. As of today’s date May 19, 2026, (L1) has created a schedule to ensure that the facility does not go overcapacity in regards to Infants in care that was sent to LPA by the due date that was discussed with (L1) along with the other proof of Plan of corrections. (L1) observed during today’s inspection (L1) within capacity limits during visit. (L1) acknowledges the Plan of correction that was worked on to ensure that facility does not go over capacity. During today’s Inspection, LPA also was able to confirm that (L1) has set up an additional Crib for Infants in care. LPA was also able to confirm that Present children have proof of Immunization's. Continued on Page 2... Page 2 Continued... During today’s inspection, LPA conducted Children Record Review for present children, Personnel Record Review and Facility Documents. LPA was able to confirm that current enrolled children had the Acknowledgement of Receipt of Licensing Reports (LIC 9224) signed and available for review in children files. LPA reminded (L1) that the (LIC 9224) must continue to be provided to upcoming enrolled families for the next 12 Months. LPA observed the Notice of Site and Facility Inspection Report provided to licensee on May 5, 2026 to be posted in home and available for review upon entrance of the facility. During today’s inspection LPA was able to clear 3 deficiencies of the 3 Deficiencies that were issued on May 5 th , 2026 during the annual inspection. Letters of deficiencies cleared were printed and provided to (L1). No deficiencies were issued during today's visit. Appeal Rights were explained and provided to (L1). A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee, Ana Bertha Santiago.
