On December 15th, 2025 at approximately 9:35am, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced annual inspection. LPA met with Lead Teacher, Hiroko Bilick. Director, Mako Garcia later showed up. LPA explained the purpose of the inspection to both. Present in the facility are Director and seven staff caring for a total of 27 preschool age children. Not all adults working in the facility are fingerprint cleared and associated (Type A citation was received). During inspection, facility was operating within teacher to child ratio on this date. Preschool program operates in two classrooms: Rainbow and Preschool. Days and Hours of operation are Monday-Friday from 8:30m-5:30pm, year-round with occasional Holiday and breaks. LPA and Director inspected the facility for Health and Safety Hazards. LPA observed each classroom to be clean and equipped with age-appropriate toys and materials. Storage for children's belongings are located inside the classrooms, labeled with each child's individual names. LPA observed facility has an installation of a fire alarm system inside the classroom as well as a fully charged, fire extinguishers. The facility has a smoke and carbon monoxide detector installed as well as emergency kits with first aid materials and emergency contact information. There are children enrolled that have prescribed medication and needs to be administered as needed. LPA observed medication to be stored in a medical box, inaccessible to enrolled children and maintained with proper documentation in the Preschool classroom. All poisons, cleaning solutions and hazardous materials are stored inaccessible to children in off limit areas or facility high shelves. Facility offers am/pm snacks. Parents have the option to bring in children\342\200\231s lunch or order lunch from Kikkoman. AM/PM Monthly menu is posted and are sent to parents. Water is available to children indoors and outdoors. Facility refills water bottles as needed. Children\342\200\231s sleeping cots are stored inside the classroom with children\342\200\231s blankets stored in their cots. Per Director, children\342\200\231s blankets are sent home once a week. Continued on Page 2... Page 2 Continued... Outdoor area appears to be clean and orderly. There are a variety of outdoor equipment that are in good working condition. Children are signed in and out of facility with parents' signatures through Brightwheel. Facility has license documentation and information properly posted and available for review. LPA reviewed facility records that included 6 children's records and 6 staff records. LPA observed children's and staff's records to be complete. There is at least one staff that has a valid CPR and First Aid certificate. All staff have an up-to-date Mandated Reporter Certificate. Last emergency drill was conducted 09/2025. Emergency drills must be conducted at least once every six months and must be properly logged and documented. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Deficiencies were cited today under CCR, Title 22, Div. 12, Chapt. 1. LPA Tapia-Mandujano informed facility representative, Mako Garcia that this report dated 12/15/25 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care. Also, LPA Tapia-Mandujano informed the facility representative, Mako Garcia to provide a copy of this licensing report dated 12/15/25 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
