What the inspector wrote

On 5/6/2025 at 1:35PM., Licensing Program Analyst (LPA), Luis Gomez met with Licensee, Day Anne Suspine. The purpose of today\342\200\231s visit was explained and was for an unannounced, annual/ random inspection. Present was the licensee and caring for 11 children. ( 5 infant-age, 6 preschool-age) During inspection, 12 child (school-age ) and 2 nd licensee, Anthony Esguerra arrived to facility. Per licensee\342\200\231s school-age child present is her daughter. Adults have criminal record clearances on file. Licensee\342\200\231s home is a two bedroom, two bathroom, one level house. Days and hours of operations are Monday- Friday, 7:30AM- 6:00PM. The areas of the home designated for childcare are: Living Room (Activity Room); Bathroom #1; Dining Area; Kitchen; Bedroom #1; and Backyard. The area designated as off-limits include Garage; Bedroom #2, and Bathroom #2. LPA inspected facility indoors and outdoors for health and safety hazards. At 1:40AM., Based on observation and interview, LPA confirmed licensee operating out of ratio, with one adult (Licensee) present with 11 children in care. At 1:45PM., the following was observed: Facility was clean, neat, with age-appropriate playthings and material available for the children. The floors, walkways, and ground surfaces were clear of any obstructions or hazards. The off-limit areas are made inaccessible with lockable doors. Accessible table, chairs, and furniture are scaled to the appropriate size. Facility materials and playthings inspected were in good repair, free of sharp corners or splinters. Fireplace in activity room has been barricaded. For napping services/ scheduled rest, LPA observed plastic mats and play pen located in facility. Play pens were equipped with properly sized mattress and tight fitted sheet. Bathroom #1 was observed clean, with faucet and toilet in operating condition. Facility was a comfortable temperature, ventilation, and lighting. Facility has telephone service; carbon monoxide/ smoke combination detectors; and fire extinguisher (2A:10BC). (REFER TO 809C, FOR CONT.) (PAGE 2) LPA inspected facility backyard area. Area was completed fenced with plaything in good repair. Facility does not have any pools; fishpond; jacuzzi; or other bodies of water. At 2:30PM., LPA reviewed facility records including the children files. The children\342\200\231s files were reviewed and included the: Identification and Emergency Information (LIC700); Consent for Medical Treatment (LIC627); Affidavit regarding liability insurance (LIC282); and Notification of Parents Rights (LIC995). At 2:40PM., Based on record review, interview, and observation, LPA confirmed licensee operating beyond limits stated on license with 12 children (5 infant-age) children in care. At 2:45PM., Based on record review, LPA confirmed LIC9227, Individual Infant Sleeping Plan missing from qualifying infant's file (C1). LPA reminded licensee to document infant napping conditions during each 15 minute review. Documentation must include child's name, date, and time. Advisory Note: Technical Violation (LIC9102TV) was issued. The licensee\342\200\231s cardiopulmonary resuscitation (CPR)/ Pediatric First Aid Certification was current, expiring: 9/21/2026. LPA reminded licensee to conducted emergency disaster drills every six months. Advisory Note: Technical Violation (LIC9102TV) was issued. Per licensee, isolation of an ill child is in Bedroom #1. Per licensee, transportation is provided via outside vehicles. LPA observed car seats (booster seat for children). Per licensee, infants are not transported. (REFER TO 809C, FOR CONT.) (PAGE 3) Licensee was reminded that all adults 18 years and over living in the home, person who provides care and supervision to children, and staff who have contact with children, including employee and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain criminal clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPA discussed safe sleep regulations with licensee and the Child Care Licensing Safe Sleep Web page at: https://www.cdss.ca.gov/inforesource/child-care-licesning/public-information-and-resources/safe-sleep as an additional resource. LPA informed licensee of the importance of checking for recalled infant devices on United States consumer Product Safety Commission (CPSC) website at http://www.cpsc.gov and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Facility was informed of the www.mychildcareplan.org site is a consumer education website that helps families obtain child care by connecting to child care providers and resources and referral agencies (R&R) throughout California. Incidental Medical Services (IMS) policy was discussed. For IMS information, see PIN 20-02-CCP. (PAGE 4) Based on today's inspection, deficiencies were cited in areas evaluated according to California Title 22, Div. 12 Chap. 3, Health and Safety Code of Regulations and cited on 809D. An exit interview including plans for correction and facility evaluation report was discussed with Licensees, Day Anne Suspine. Licensee signature of this form acknowledges the receipt of these documents. Licensee was issued Type \342\200\234A\342\200\235 violation and was advised to provide a copy of the \342\200\230Facility Evaluation Report\342\200\231 and all Type \342\200\234A\342\200\235 Deficiencies cited, to parents and guardians of children currently enrolled in care and to parents of newly enrolled children during the next 12 months. Signed LIC 9224, \342\200\230Deficiency and Acknowledgment of Receipt of Licensing Reports; shall be maintained in all children's files. During exit interview, licensee confirmed no registered sex offenders are living in the facility, and LPA completed RSO profile. Notice of site visit was given and must remain posted for 30 days.
