What the inspector wrote

On August 20, 2025 at approximately 1:45PM Licensing Program Analyst (LPA) Katie Krenn arrived at the facility to conduct an unannounced case management visit regarding an unusual incident that the facility self-reported. LPA met with Director, Elizabeth "Omi" Linsdey and explained the purpose of today's visit. There were 29 preschool children being supervised by ten teachers. The facility remains within its capacity limits and meets the required ratios for today\342\200\231s visit. The program operates Monday through Friday from 7:30AM - 5:30PM. LPA observed that all required documents, such as the facility license, notification of parents\342\200\231 rights, personal rights, car seat laws, and earthquake preparedness checklist were displayed and visible to the public where children are dropped off and picked up by parents and caregivers. The reported incident happened on August 7, 2025 and was properly reported to the licensing office. The facility met the requirements for reporting. On that day, at approximately 4:00PM a child was unsupervised for approximately three minutes. The facility has made adjustments to their end of the day protocol in order to ensure that all children are supervised at all times. The facility will also have a training at their next faculty meeting. To improve the quality and value of the inspection process, a survey will be sent to the email address provided. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. A Type B Citation was issued during this visit. Exit interview conducted and report was reviewed with the director, Elizabeth "Omi" Linsdey.
