What the inspector wrote

On November 12, 2025 at approximately 8:00AM, Licensing Program Analyst (LPA) Katie Krenn arrived at the facility for an unannounced case management visit, regarding an unusual incident which occurred on October 14, 2025, that the facility self-reported. LPA met with Director, Elizabeth "Omi" Linsdey (D1) and explained the purpose of today\342\200\231s visit. All Five @ Belle Haven has two licenses at this location. For the preschool license, there nine staff supervising 41 preschool children in three classrooms. The program operates Monday through Friday from 7:30AM - 5:30PM throughout the year with short winter and summer breaks. LPA and D1 toured the facility. D1 showed LPA the area of the playground where the incident occurred and described the incident as they understood it. As a temporary solution the object that the child had injured themselves on had been moved to a different position. LPA and D1 discussed ideas for a longer-term solution. LPA met and interviewed the child, C1 that was injured in the incident. LPA met and interviewed the staff, S1 that observed the incident. continued on page 2 Page 2 Based on observations and interviews LPA did not identify any violations of the title 22 regulations regarding this incident. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. No citations were issued during today's visit. Exit interview conducted and report was reviewed with the director, Elizabeth "Omi" Linsdey.
