What the inspector wrote

On May 15, 2026, at approximately 2:00PM, Licensing Program Analyst (LPA) Jonathan Tse conducted an unannounced case management visit at the facility. LPA met with Site Supervisor, Sue Fosgett, and explained the purpose of the visit. Present during the visit were 8 staff members and 55 preschool age children. The facility self-reported an incident that occurred on April 29, 2026, where a child (C1) got their arm stuck in a door jam. The fire department was called and C1 was freed from the door jam. Parents were notified of the situation. C1 was taken to seek medical attention, and was assessed to not require an x-ray. C1 returned to the facility the next day with no issues. The facility has placed an order for metal pieces to fill in the door jams of all classrooms (6 doors total). There is currently a delay in installation of the metal pieces. As a result, the facility is now keeping doors closed while children are playing outside to ensure that similar incidents do not occur. No similar incidents have occurred since April 29, 2026. The facility is in compliance with reporting requirements. This report was sent to the facility's email. No deficiencies were cited during today's visit on May 15, 2026. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with Site Supervisor, Sue Fosgett.
