What the inspector wrote

On May 13, 2025, at approximately 1:30PM Licensing Program Analyst (LPA) Katie Krenn conducted an unannounced, annual visit. LPA met with Site Director, Bernadette Barte and explained the purpose of the visit. Present during inspection, in addition to the site director were 20 school age children (Ranging from TK \342\200\223 5th Grade), three teachers and three teacher\342\200\231s assistants. All staff present have fingerprint clearance. One staff present was not associated with the facility. Childcare Areas: Classrooms four & nine. Basketball court, handball court, sports field, older child outdoor playground and younger outdoor child playground. During the school year the program operates the morning program Monday through Friday from 7:00AM to 8:30AM. The after school program operates Monday, Wednesday, Thursday, and Friday from 3:00PM - 6:30PM. On Tuesday the program is from 2:00PM to 6:30PM. On minimum days the program begins when the children get out of school. The program offers full day on breaks and some holidays, when the school is closed. LPA observed that all required documents: such as the facility license, notification of parents\342\200\231 rights, personal rights, car seat laws, earthquake preparedness checklist, and emergency disaster plan, were available for review in classroom four. LPA advised the site director to add these postings to classroom nine as well. Site Director confirmed that she understood. All children are signed in by champion staff. The Champion program operates on an active elementary school site. The school age program is exempt from requirements for indoors and outdoors space square footage, fencing and toilet/sink ratios. No measurements were taken during LPA's visit. continued on page 2 page 2 According to the Site Director, none of the children have any known allergies that would require an epi-pen or take any medications. The program is nut free. LPA observed the classrooms to be clean, equipped with age-appropriate furniture and educational materials. Flooring in classroom includes carpet that was observed to be clean and free of stains and tile that had minimal scuff marks and small debris. Classroom includes a fully charged fire extinguisher and an integrated fire alarm system implemented by school district. LPA observed and tested carbon monoxide detectors and found them to be functioning. LPA observed that children\342\200\231s belongings are stored on shelves in the classroom. LPA inspected the emergency backpack kit with first aid materials and emergency contact information. The backpack did not include scissors. Site Director will add scissors. Per Site Director, the last emergency drill was conducted in March 2025. Emergency drills are conducted at least every six months at this facility and LPA observed that they are properly logged and documented. The facility provides snacks. LPA observed a menu of snacks that included healthy options, such as fresh fruits, vegetables, and whole grains. Drinking water is available for the children both indoors and outdoors. The outdoor play area is surround by a fence that is at least 4 feet tall. Under the climbing equipment for the older child playground is a rubber-based flooring suitable to cushion falls. Under the climbing equipment for the younger child playground is artificial grass suitable to cushion falls. LPA observed that the children had access to sports balls and other items for field sports. LPA reviewed the scheduled activities which included suitable activities such as outdoor organized or free play, snack time, as well as time for silent reading and homework. LPA inspected the bathrooms for the children and found them to be clean and functional. Children have separate bathrooms for each gender. There are separate bathrooms available for staff. continued on page 3 Page 3 LPA reviewed the files of five children. Some documents were incomplete. LPA reviewed this information with the Site Director. The Site Director confirmed that they would ensure that the files were updated with the missing information. The vaccination records are kept by the school district. LPA reviewed four staff files and found that two staff were missing the health screening reports. Site Director remembered having them at some point, but they are missing from the file. One staff was missing complete transcripts. LPA verified that at least one staff had current valid CPR and First Aid Certification. All the files reviewed had current Mandated Reporter Training Certifications. LPA saw that the staff had complete vaccination records. To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Site Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. No deficiencies were cited during today's inspection. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Site Director, Bernadette Barte.
