What the inspector wrote

On November 6, 2024 at approximately 11:30am, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, annual inspection. LPA met with licensee/director, Ana Acevedo, and assistant director, Daysi Adqui, and explained the purpose of the visit. Present during LPA's visit included 2 teaching staff (including assistant director) and 10 preschool age children. Teaching staff have fingerprint clearance on file. With licensee/director, LPA inspected preschool program, indoors and outdoors, for health and safety hazards. Hours of operation are Monday through Friday 7:00am to 5:30pm. LPA observed preschool classroom to be clean and appropriately ventilated. Flooring in classroom includes carpet and rugs that are clean, free of stains. There are a variety of children's toys, materials and furniture that were observed to be age appropriate and in good working condition. Per assistant director, toys and materials are sanitized multiple times a week. Classroom is set up in areas that includes arts and crafts, reading and building materials. LPA did not observe accessible cleaning solutions, chemicals or poisons to be stored in classroom. LPA observed cleaning solutions to be stored in classroom's high shelves and/or in off limit area, that is locked. Facility is equipped with a fire alarm system, fully charged fire extinguisher, smoke detectors, and carbon monoxide detector. Carbon monoxide detector was tested during LPA's visit, which was observed to be working. LPA did not observe poisonous materials in accessible drawers in classroom. Storage for children's belongings are located in the lobby area of facility, labeled with children's individual names. In the lobby area, licensing documents are properly posted and available for review. Restroom for children's use was observed to be in working condition. Sink accessible to children does not provide water that is warmer than 120 degrees Fahrenheit. LPA observed children's restroom to be equipped with appropriate sanitation materials. There is a diaper changing table that is within arm's reach to a sink. Per licensee, children's families provide diapers and wipes. LPA observed children's diapers and wipes to be appropriately stored, labeled with children's names. Dirty diapers are disposed of in a garbage bin with a tight fitting lid. (Continue Report on Page 2...) (Continued, Page 2...) Outdoor area is entirely enclosed and fenced. Outdoor area includes outdoor toys and materials that are in good condition. There is a play structure with resilient padding underneath. Per licensee, outdoor toys and materials are sanitized daily. LPA did not observe any pools, spas or bodies of water on site. There are stairs in the outdoor area that are made inaccessible to children. Children sleep in the classroom on cots. Pillows and blankets are provided by preschool program. Per assistant director, pillows and blankets are laundered weekly. LPA observed children's pillows and blankets to be labeled with their individual names. Program does not currently provide a food service. Children bring their own food to facility daily. LPA observed children's lunch boxes to be stored in their individually labeled cubbies. Per licensee, staff will heat children's meals, however, no other food service is provided. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Drinking water is made available for children both indoors and outdoors through children's individual water containers. Per licensee, children's water containers can be refilled on site through purchased water bottles. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. (Continue Report on Page 3...) (Continued, Page 3...) In the lobby area, children are signed in and out of program through written documentation. LPA observed sign in/out sheets specify the date and time the children are signed in, however, do not include legal signatures of the individual bringing the child to facility. LPA observed sign in/out sheets specify the individual who brings the child to and from the facility, completed by staff. LPA reminded director and assistant director, individuals who bring children to and from the facility must sign the child in and out with their written legal signatures Emergency disaster drills are conducted monthly and are appropriately documented. Last disaster drill was conducted 10/15/2024. LPA reviewed 7 random children's records. Children's records have emergency contact information and immunization records. LPA reviewed all teaching staff's files. Teaching staff present have record of their required immunizations available for review. All teaching staff have a current CPR/First Aid certification. LPA did not observe a current Mandated Reporter certification for any staff member. LPA reminded director Mandated Reporter training must be completed by all staff every two years. LPA provided link to complete training. LPA also provided Entrance Checklist (LIC125) to director during visit. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA issued two Type B citations for children's incomplete sign in/out records and incomplete Mandated Reporter certifications. A plan of correction was discussed. Appeal Rights were provided during visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with licensee/director, Ana Acevedo.
