What the inspector wrote

On November 18, 2024 at approximately 11:45am, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, plan of correction (POC) visit. LPA met with licensee/director, Ana Acevedo, and explained the purpose of the visit. Present during LPA's visit included licensee/director, 1 teaching staff, and 9 preschool children. On November 6, 2024 at approximately 11:30am, LPA conducted an unannounced, annual inspection to facility. During annual inspection, LPA observed sign in/out sheets did not have legal signatures of the individual bringing children to the facility. LPA observed sign in/out sheets were completed by staff. Facility was issued a Type B citation. LPA also observed that no staff present during annual inspection had a completed Mandated Reporter certification. Facility was issued a second Type B citation. A plan of correction was discussed with licensee/director. During today's visit, LPA reviewed sign in/out records since LPA's visit on November 6, 2024. LPA observed sign in/out records are completed for children. Sign in/out records include the time the child was signed into the program and signed out of the program. LPA observed sign in/out records to also include the legal signatures of the child's authorized representatives. LPA observed completed sign in/out records from November 7, 2024 through today's date. LPA also observed teaching staff to have a current Mandated Reporter (AB1207) certifications. LPA reminded licensee Mandated Reporter certification must be renewed every two years by all staff working with children. LPA observed notice of site visit provided during annual inspection to be posted and available for review. Deficiencies cited November 6, 2024 are cleared during today's visit. LPA provided POC letter during visit. No deficiencies were issued during today's visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with licensee, Ana Acevedo.
