Correction check·October 29, 2025

On 10/29/2025, at approximately 3:20PM, Licensing Program Analyst (LPA) Alvarado conducted an unannounced Plan of Correction (POC) visit at the facility. LPA met with the Licensee Christina Houlihan(L1), Purpose of the inspection was explained with (L1). Present in the facility was (L1), assistant and two children (one infant and one preschool age child). All adults present in the home have fingerprint clearance and are associated to the facility. LPA along with (L1) inspected the facility for any health or safety hazards indoors and outdoors. During the Annual inspection visit on 9/30/2025, the Licensee received one citation regarding 15Min-Sleep Check Logs. LPA Alvarado observed during the annual inspection that Infant sleep logs were not being continued to be maintained. As of today\342\200\231s inspection, LPA during record review observed that Infant sleep logs are being maintained for infants under 24Months. One out of the one deficiency cited on 9/30/2025, was cleared today on 10/29/2025. The Letter of Deficiency Citation Cleared was provided to (L1). No deficiencies were cited during today's visit on 10/29/2025. Appeal rights were provided to the licensee, Christina Houlihan. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee, Christina Houlihan.
