Inspector’s narrative

What the inspector wrote

On 8/27/2025, at approximately 10:00AM, Licensing Program Analysts (LPAs) Alvarado and Tse conducted an unannounced case manegment visit at the facility. LPAs met with facility Director, Holly Anderson (D1) and explained the purpose of the visit. Present during the visit was D1, 15 teachers, and 33 infants. On August 14, 2025, the facility self-reported an incident where a child was fed an incorrect bottle of breast milk. This incident was reported to the child's parents. The facility was compliant with reporting requirements. During the visit, LPAs observed that milk bottles were labeled with each child's name and color coded. Based on discussion with D1, there was a miscommunication between teachers present. LPAs reviewed the facility's feeding policy. The facility has conducted an internal investigation and placed the teachers involved on leave. Trainings were conducted and meetings were held with all infant staff members. See LIC 809-D for deficiencies being cited today on 8/26/2025 under the California Code of Regulations, Title 22, Division 12, Chapter 1, regarding infant feeding plans. Appeal rights were provided to Facility Director, Holly Anderson. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee, Holly Anderson.
