Inspector’s narrative

What the inspector wrote

On June 3, 2025 at approximately 10:00AM, Licensing Program Analyst (LPA) Katie Krenn conducted an unannounced continuation annual inspection. LPA met with staff and explained the purpose of today's visit. When LPA arrived there were three staff caring for four infants, six preschool children, and one school age child. Licensee arrived 15 minutes into the visit. The licensee has a large license and is operating within the capacity limits and ratios. All adults living and working in the home have finger print clearance. Licensee rents home, which is a 3 bedroom, 1 bathroom, single level duplex house. Licensee lives alone. The hours of operation are Monday - Friday from 8am - 5:30pm. Daycare areas are: Living Room, Dining area, Bedroom #1, #2, and #3, Bathroom #1, and Back yard. Off limit areas are: Kitchen, Laundry area, (detached) Garage, and Front yard. All off limit areas are properly barricaded. LPA observed that the facility license, the Notification of Parental Rights poster, emergency disaster plan, and Earthquake Preparedness Checklist were visible to the public. LPA inspected the indoor and outdoor daycare areas to identify health and safety hazards. LPA reviewed the emergency disaster drill log and found the most recent emergency disaster drill that had been recorded was conducted in April 2025. LPA observed that it was documented. The home had a fire extinguisher, a first aid kit, and a new combination carbon monoxide and smoke detector 9in the living room and each bedroom. The licensee's CPR/First Aid certification expires in September 2025. The three helpers all had First Aid CPR certification expiring in April 2027. continued on page 2 page 2 LPA observed that the home's interior was clean and orderly, with proper heating and ventilation. Disinfectants, cleaning solutions, poisons, and other hazardous items were stored in areas inaccessible to children. The bathroom was clean, fully operational, and equipped with suitable toileting equipment and sanitation products. LPA observed that the daycare areas had age-appropriate toys, furniture, and educational materials. The heater was covered. A designated area was provided for storage of the children's clothing and personal belongings. Electrical outlets were fitted with child safety covers to prevent children's access. According to the licensee, there are no firearms or weapons on the property. The backyard is surrounded by a fence and features a variety of outdoor play equipment, all of which are in excellent condition. Artifical grass is underneath the climbing equipment to cushion falls. LPA did not observe any pools, spas, or bodies of water on the property. The licensee stated she currently provides breakfast, lunch, and snacks for the children. The licensee stated that there are no documented allergies among any of the enrolled children. The licensee stated that they are not administering any medications at this time. The facility offers different options for children to rest or nap, including cots and play yards. LPA observed that sheets and blankets were available for the children. Infant mattresses are firm and equipped with appropriately sized sheets. All play yards are well-maintained and free of loose items that could pose safety risks. LPA reviewed the files of five children in care and found that all the necessary documentation was in order for those five children. LPA noticed that two of the children needed updated vaccination records. During the visit licensee contacted the parents to request updated vaccination records. One school age child in care did not have a file. LPA reminded licensee that all children need to have a complete file available for review. LPA also reminded licensee that all children in care are required to be vaccinated unless they have a documented medical exemption. Personal or religious exemptions are not permitted. continued on page 3 page 3 LPA reviewed the files of the licensee and three helpers. LPA found that all of the files reviewed had all the necessary documentation, including current CPR and First Aid certification and current Mandated Reporter Training. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
