What the inspector wrote

On 10/29/2025, Licensing Program Analysts (LPAs) Wadhwa and Tso conducted an unannounced annual visit. LPA met with the Licensee, Yan Yu Zhao, and explained the purpose of the visit. Today\342\200\231s visit included the licensee along with two assistants caring for 7 children (4 infants and 3 preschool age children). The licensee has a large license and is operating within ratio and capacity limits on this day. All adults working on this day have fingerprint clearance on file. Hours of operation are Monday to Friday 8:00 AM to 6:00 PM. Licensee cares for children in the age range of newborns to to 5 years old. LPAs conducted a health and safety inspection inside and outside the home. The home is a single-family one level home. Entrance to facility is through a side gate located in the front of the house. DAY CARE AREAS are the living room (main classroom), bedroom #1 (nap room), bathroom #1 and fully enclosed backyard. OFF LIMIT AREAS are the kitchen, bedroom #2 and bedroom #3. Off limit areas are made inaccessible with child safety door handles or child safety gates. Per licensee, she has 11 enrolled children in her daycare. LPAs reviewed the facility\342\200\231s children's roster and found it to be current. LPAs observed that all required documents, such as the facility license, the Notification of Parental Rights, and the Earthquake Preparedness Checklist, were displayed and visible to the public. Continued on Page 2 Page 2 The most recent emergency disaster drill was conducted on 10/23/2025, and the LPA observed that it was properly documented. LPAs reviewed the emergency disaster drill log and found they were conducted at least once every two months. LPAs inspected the indoor and outdoor daycare areas to identify health and safety hazards. The home had a fire extinguisher, a first aid kit, multiple carbon monoxide and smoke detectors. LPAs tested the carbon monoxide and smoke detectors and found them to be in working condition. LPAs observed that the home's interior was clean and orderly. Disinfectants, cleaning solutions, poisons, and other hazardous items were stored in areas inaccessible to children. The bathroom was clean, fully operational, and equipped with suitable toileting equipment and sanitation products. LPAs observed that the daycare areas had age-appropriate toys, furniture, and educational materials. The fireplace was covered with a screen and made inaccessible to children. A designated area was provided for permanent or portable storage of the children's clothing and personal belongings. Electrical outlets were fitted with child safety covers to prevent children's access. The licensee stated that no firearms or weapons were present on the property. The backyard is surrounded by a fence and features a variety of outdoor play equipment, all of which are in good repair. LPAs did not observe any pools, spas, or bodies of water on the property. The licensee stated she provides the children with breakfast, lunch, and snacks. The licensee stated no documented allergies among any of the enrolled children. The facility offers different options for children to rest or nap, including cribs and cots. LPA observed that sheets and blankets were available for the children. Per licensee, parents provide the children with blankets and sheets, which are sent home weekly for laundering. Infant mattresses are firm and equipped with appropriately sized sheets. All cribs are well maintained and free of loose items that could pose safety risks. Continued on Page 3 Page 3 LPAs reviewed the files of five children and found them to be complete. LPAs also reviewed the 15-minute sleep records of four infants for this week and confirmed they were properly documented. LPAs reviewed staff files including the licensee\342\200\231s file and confirmed all staff have the required immunizations on file. Licensee\342\200\231s Mandated Reporter Training certification is current and will expire in July 2026. Licensee also has a valid CPR certification. One staff was found be to missing their Mandated Reporter Training Certificate. Licensee was advised that Mandated Reporter Training is now available in Chinese languages and must be completed by all employees. Advisory Note: A Technical Advisory (LIC9102 TA) was issued today. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Continued on Page 4 Page 4 During the exit interview, the licensee, Yan Yu Zhao confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
