What the inspector wrote

On January 22, 2026, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced Case management due to deficiencies observed during a complaint investigation. LPA met with Site Supervisor Michelle Salinas Quintanilla and explained the purpose of the inspection. Facility operates a preschool program with a toddler component. Present in the facility today were Site Supervisor and 2 staff supervising 14 children (10 preschooler and 4 toddlers). LPA verified that all adults present during today’s inspection have criminal background clearances and are associated to facility. During today’s visit, LPA conducted staff file reviews. LPA observed staff files to be missing completed Health Screening Report (LIC503), as this poses a potential health, safety and personal rights risk to children in care, a type “B”’ deficiency was cited. Please see page LIC809D for type “B” deficiency issued today under California Code of Regulations, Title 22, Division 12. . A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements may result in an immediate civil penalty of $100. This report is public and can be reviewed. Exit interview was conducted and report was reviewed with Site Supervisor, Michelle Salinas Quintanilla. Appeal Rights were provided.
