What the inspector wrote

On November 18, 2025 @ approx. 9:20am, Licensing Program Analyst (LPA) Maria Olguin-Leon and Licensing Program Manager (LPM) Marie Rodriguez conducted an unannounced, plan of correction (POC) visit in conjunction to deliver a scheduled office meeting letter to Licensee. LPA and LPM met with Licensee Edelmira Cardenas and explained the purpose of the visit. Present during today's visit was Licensee and helper caring for 7 children (4 infants and 3 preschoolers). Facility is operating within ratio of staff/children’s capacity. On November 6, 2025, LPAs Olguin-Leon and Tapia-Mandujano conducted an annual visit, a case management visit and a complaint investigation visit to deliver findings of complaint allegations. During visit, Licensee was cited three “A” deficiencies for unfingerprinted adult, cribs or play yards shall be free from all loose articles and objects and children’s personal rights violations. Licensee was also cited three type “B” deficiencies for broken toys and soiled playpens, maintaining an updating children’s roster for three years for all enrolled and disenrolled children, and maintaining all personnel records which shall be kept for at least three years following termination of employment/volunteer work. A POC was developed with Licensee Edelmira Cardenas. During today's visit, LPA and LPM observed signed and completed Acknowledgement of Receipt of Licensing Reports (LIC9224) for 7 enrolled children. Per licensee, copies of all three reports were provided to all parents of currently enrolled children. LPA and LPM observed a "For Parent Review' binder with licensing reports available at entrance of daycare . Cont. page 2... Page 2... The three type A deficiency and three type B deficiencies cited on November 6, 2025, have not been cleared as of this date. LPA reviewed deficiencies with Licensee and Licensee will submit letter of acknowledgment that Licensee has read and reviewed all regulations cited and Licensee’s plan to ensure regulations are followed. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee, Edelmira Cardenas.
