What the inspector wrote

On December 7, 2022 at 3:40 PM, Licensing Program Analyst (LPA) Cowan met with Site Director, Stephanie Rubio, for an unannounced subsequent complaint inspection. The purpose of inspection was explained to site director. Present in the facility is site director, 1 staff caring for 19 children. During the course of investigation, LPA interviewed director, managers, and staff. Based on the information obtained from interviews, a child has been left in soiled cloth for a period of time as it was known to staff. Staff did with hold food from child until child ate their vegetables. Staff member did raise their voice at a child. Lastly, staff told child that they will have to go to the infant classroom since child was misbehaving. Based on LPA’s interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegations is founded to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division & Chapter number), are being cited on the attached LIC 9099D. Allegation is determined to be Substantiated This report and appeal rights are reviewed with site director and a Notice of Site is given this day and shall remain posted for 30 days.

    101223 Personal Rights(a) The licensee shall ensure that each child is accorded the following personal right (1) To be accorded dignity in his/her personal relationships with staff and other persons.This requirement was not met as evidenced by: Based on interviews, a child has been left in soiled cloth for a period of time as it was known to staff.
