(PAGE 2) Based on evidence obtained, LPAs were unable to determined is facility did not properly addressing lice outbreak. During interviews, director reported after confirmation of lice, a deep cleaning of infant classroom was completed. Per director, standard cleanings are conducted every weekly, included the disinfecting of toys, playthings, and accessible items. Based on evidence obtained, LPAs's were unable to determine if facility does not ensure staff are in good health to perform their assigned tasks. Per director, classroom staff are required to call in sick when necessary. Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Director, Yasha Nasiripour. Appeal Rights were provide to facility.
