C1 = the child On May 28, 2026 at approximately 12:45 PM, Licensing Program Analyst (LPA) Tso conducted an unannounced, case management visit. LPA met with the Director, Lisa Aguirre, and explained the purpose of the visit. Present in the facility is Director, 4 staff, and 15 children in care. . The case management visit is regarding an unusual incident that occurred on May 15, 2026 (Friday). Facility self-reported incident to CCLD on May 18, 2026. On May 15, 2026 (Friday), C1’s right finger was caught in the doorway as the door was being shut. LPA interviewed with the Director. C1 had not been absent from the facility and returned to the facility next Monday, May 18, 2026. LPA observed 3 doors were installed the door guards to prevent the reoccurrence. There was no sufficient evidence for any violations. There were no deficiencies cited at this time under CCR, Title 22, Div. 12. A copy of today’s report was given to the Director. Notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Lisa Aguirre.
