What the inspector wrote

On April 9, 2024, Licensing Program Analysts (LPAs) Ly and Gil, arrived at the facility unannounced to close the complaint investigation into the above allegations and met with Director Sophin Francis. There are 41 children in attendance today with the present of 8 staff including the Director. Based on information obtained during the course of this investigation through interviews and observation, it is determined that allegations Staff do not ensure toilets are kept in good repair and Staff do not keep the facility free of odor did occurred. The preponderance of evidence standards has been met, therefore, the above allegations are found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Type B deficiencies are being cited on the attached LIC9099D. A copy of this report and appeal rights were discussed and left with Director whose signature on this form confirm receipt of the report. Notice of Site Visit was provided. Notice to remain posted for 30 days.

This requirement was not met as evidenced by: Based on LPA's observations, licensee did not comply with this regulation in that the facility had what seemed have a strong unpleasant ordor. This poses a potential health and safety risk to children in care.

    101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met evidence by: Based on LPA observation on April 18, 2024, two toilets were in good repair and plastic bags were covered to prevent children from using. This poses a potential health and safety risk to children in care
