What the inspector wrote

On April 24, 2025, at approx. 1:00 pm Licensing Program Analysts (LPAs) Maria Olguin-Leon and Leslit Tapia-Mandujano conducted an unannounced, annual inspection. LPAs met with Director Connie Rosas and explained the purpose of the inspection. Present in the facility today were, Director, 1 staff, and 8 preschool children. All adults working in the facility have criminal background clearances and associated to facility. Facility is operating within capacity and in compliance with staff/child ratio. Facility operates a preschool after school program. Hours of operation are Monday-Friday 2:40 pm \342\200\223 6:00 pm, year-round. On December 5t, 2023, LPA Tapia-Mandujano conducted an inspection, where it was observed that facility was operating in classroom #4 with both preschool and school age children comingling. During today\342\200\231s visit Director stated preschool and school age children were comingling in classroom #4. During visit Director separated the school age children into classroom #2. Type A deficiency was cited. LPAs and Director toured the facility for health and safety hazards. Facility operates a preschool after-school program and school age afterschool program on the Campus of Sacred Heart School. Facility is licensed for classrooms #1-4, #606 and #608. Preschool after school program is currently only operating in classroom #4. Classroom has one restroom with a working toilet and a sink with no running hot water. Classroom is clean and orderly with proper temperature and ventilation. There are plenty of age-appropriate toys, child size furniture, learning materials, books, indoor dramatic play area, all in good repair. Children do not nap in preschool after-school program and per Director are allowed rest time. Classroom has storage cubbies available for children to store personal items and belongings. There is a kitchenette in classroom with two sinks and a microwave. There is also a refrigerate in the classroom. There are also two separate sinks located in classrooms. There is a separate restroom for staff usage. All trash containers in restroom and in classroom have tight fitted lids. All cleaning products, chemicals and toxins are stored and locked in cabinet and inaccessible to children. Cont. Page 2... Page 2... Outdoor playground includes an age-appropriate slide/climbing play structure, sand box area with rollout cover, and basketball courts. LPAs observed a grass area and asphalt area for children to play. LPAs observed tanbark cushioning under large play structure to cushion falls. The entire playground is completely enclosed with a 5 ft. metal fence. LPAs observed sunshades for provide shade to children in care. Sacred Heart School campus has a swimming pool on the opposite far end of campus where children had no access. Per Director, there are no weapons or firearms on the premises. Facility provides one snack to children in care. Monthly snack menus are properly posted. LPAs observed kitchenette area to be clean, and food stored properly to prevent contamination. Children bring their own labeled water bottles and are refilled using classroom Brita water pitcher. First aid kit is fully stocked with medical supplies. Incidental Medical Services were discussed. Sick children are isolated in office away from other children. LPAs reviewed sign in/out sheets; staff sign in preschool children using a CSR (child\342\200\231s supervision record) and parents sign children out on sign out sheet. Classroom is equipped with fully charged fire extinguisher. Type B deficiency issued for missing carbon monoxide detector. Smoke detector is integrated into fire alarm system. Facility is equipped with a landline phone and staff use walkie talkies for communication. LPAs reviewed 5 children\342\200\231s records 3 staff records. Technical violations issued for missing immunization records and missing physician reports for children\342\200\231s records. The facility has all necessary paperwork posted, Facility License, Notification of Parent's Rights, Notification of Personal Rights, Car Seat Law, monthly snack menus, and Emergency Disaster Plan. Type B deficiency was issued for not conducting emergency drills within the last 6 months. . Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. Per Director, facility was built after 2010. Type B deficiency will be cited. Director is aware that all staff is required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com . LPA encourages the director to frequently visit our website at www.ccld.ca.gov for licensing regulations and new updates. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Also, LPAs Olguin-Leon and Tapia-Mandujano informed the Director Connie Rosas to provide a copy of this licensing report dated April 24, 2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100 Exit interview conducted and report was reviewed with the Director, Connie Rosas.
