What the inspector wrote

On 11/14/2025, at approximately 1:00PM, Licensing Program Analyst (LPA) Jonathan Tse conducted an unannounced annual continuation visit at the facility. LPA met with Director, Xiaotian \342\200\234Tara\342\200\235 Yu, and explained the purpose of the visit. Present during the visit was the Director, 2 staff members, and 4 children. This is an annual continuation visit from an annual visit initially conducted on 11/6/2025. This facility is a combination center, with a preschool license (414004479) located in the same address. LPA and Director inspected the facility for any health or safety hazards. There is at least one fully charged 2A10BC fire extinguisher present. Carbon monoxide and smoke detectors were observed to be present. Emergency exits are clearly marked and visible. Poisons, cleaning detergents, and other chemicals are stored inaccessible to children in care. Outlets are covered or blocked by furniture when not in use. There are heating and ventilation controls for the comfort and safety of children in care. During the initial annual visit on 11/6/2025, LPA observed the following: A door stopper present on the floor protruding out of the floor without any barriers or barricades, a baby bouncer present in the infant napping area, a play yard in the napping area, and sleep logs not being maintained. During today\342\200\231s visit, LPA observed that furniture has been placed to cover the door stopper, mitigating risk of tripping. The baby bouncer was removed during the visit on 11/6/2025 and was not observed to be present during today\342\200\231s visit. Director stated that the play yard is now being used exclusively for play purposes, not for napping. LPA observed that sleep logs were being maintained and verified that they were being logged appropriately. Continued on Page Two Page Two Toys and learning materials are age-appropriate. Furniture is age-appropriate and free of rough or sharp edges. Children\342\200\231s bottles and meals are labeled with their names. Food and milk were verified to be labeled and not expired. Cribs and mats are available for children depending on their developmental needs. LPA observed the outdoor play area to contain age-appropriate toys and equipment. LPA advised that chipped paint be made inaccessible to children in care. Director stated that they would block it with a piece of furniture or equipment. There is resilient padding in the form of artificial turf. There are no firearms or weapons stored in the facility. There are no swimming pools or other similar bodies of water present. LPA reviewed 3 staff files, 5 children\342\200\231s files, and facility records. There is at least one staff member with current Pediatric First Aid/CPR training present. Mandated Reporter training is current for all staff files. Staff qualifications and immunization records are available for review. Based on review of children\342\200\231s files, LPA observed that 3 out of 5 children were between 2 and 3 years of age. Director stated that these children were kept in the infant program while transitioning to the preschool program. LPA advised that children can only be kept in the infant program for developmental needs. These needs must be documented and kept available for review. Children\342\200\231s files were observed to be complete. Infant sleep logs are maintained and accessible for review. The facility last conducted an emergency drill on 8/27/2025. LPA advised that required postings shall be kept posted in the entranceway of the facility. LPA referred Director to the Child Care Center Entrance Checklist (LIC125). Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Continued on Page Three Page Three Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee [or facility representative] to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ). This CCC completed testing has no lead exceedances. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP.
