What the inspector wrote

On June 4, 2026, at approximately 1:00PM, Licensing Program Analyst (LPA) Jonathan Tse conducted an unannounced case management visit at the facility. LPA met with Executive Director, Nicholas Puccinelli (S1), and explained the purpose of the visit. Present during the visit was the Executive Director and 2 staff members. There were no children in care at the time of the visit. This facility is located on the premises of White Oaks Elementary School. The facility submitted a request to add Room 5 to the licensed rooms and remove Room 10. Licensed capacity will remain the same. Fire clearance was granted on May 28, 2026. LPA and Executive Director inspected Room 5 for any health or safety hazards. Room 5 is used by the elementary school during normal school hours. The facility will have access to Room 5 after dismissal of elementary school students. Room 5 was observed to contain age-appropriate furniture. Per S1, cubbies will be rolled in on a daily basis to allow students to store their belongings. LPA observed there to be a fully charged fire extinguisher that is at least size 2A10BC present. LPA advised that a carbon monoxide detector, outlet covers, and other furniture as needed be present in the room. S1 stated that he understood. The outdoor play area was inspected. Age-appropriate equipment and materials are provided by the facility. Drinking water is provided by the facility, although children are encouraged to bring water bottles from home. Another licensed school-age facility operates on the same premises, and a schedule was provided to LPA that allows exclusive use of the outdoor play area by either program. Continued on Page Two Page Two Prior to the addition of Room 5 and the removal of Room 10 from the license, the following items are required: 1) Documentation of installed outlet covers in Room 5 2) Documentation of a functioning carbon monoxide detector and smoke detector in Room 5 3) Documentation of Room 5 being furnished with additional age-appropriate toys and equipment 4) Review of the facility's planned outdoor play schedule No deficiencies were cited during today's visit on June 4, 2026. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with Executive Director, Nicholas Puccinelli.
