What the inspector wrote

Licensing Program Analyst (LPA) Andrea Medlin met with facility representative for this plan of correction visit established on 11/1/2023. Purpose of visit explained. There are 4 children present during the visit; 1 school aged and 3 infants. The following previously cited deficiencies are observed to be corrected and cleared today: HSC 1596.8662(b)(1) - On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met based on record review. The two helpers do not have child abuse training. This is a potential health and safety risk. Licensee has has current mandated child abuse reporter training (exp. 3/29/2025); helper Rosa Amado Sanchez now has current certificate (exp 11/2/2025); helper Ingrid Amado Sanchez has current certificate (exp.11/16/2025). Section 102417- Children's files are incomplete. See LIC 857 - Children's Record Review for a list of what is missing in each child's file. This requirement is not met based on record review and incomplete/missing forms in the children's files. This is a potential health and safety risk. The files have been reviewed and are now complete. (Continued on next page 809-C) Section 102425(c)(1)(2) - INFANT SAFE SLEEP: An Individual Infant Sleeping Plan [LIC 9227 (3/20)] shall be completed for each infant up to 12 month of age the provider has in care and maintained at the facility in the infant\342\200\231s file. This requirement is not met as none of the infant's files have a safe sleep plan. There are currently no infants in care under the age of 12 months, but the form has been provided and explained how to fill out the form with parents. This report is reviewed with facility representative and a copy of this report must be made available for public review upon request. Notice of site visit provided and shall remain posted for 30 days.
