What the inspector wrote

On 12/2/2025, at approximately 2:40PM, Licensing Program Analysts (LPA) Alvarado conducted an unannounced Plan of Correction (POC) visit at the facility. LPA called the number on file and spoke to the licensee, who stated she was on her way to the facility. At approximately 2:47pm, LPA met with Licensee at Gretel Avendano(L1) and explained the purpose of the visit. Present during the visit was (L1) and assistant supervising 5 children. The facility is a large license and is operating within capacity limits and ratio during today\342\200\231s inspection. All adults present have fingerprint clearance and are associated with the facility as of todays inspection. During the Annual inspection on 9/9/2025 LPA cited four Type B deficiency regarding Personnel Records and Physical plant. Plan of correction and due dates were discussed and agreed upon with (L1). During todays visit, LPA conducted Record review and observed that the Mandated Reporter Trainings are current for both Licensee Expiring 11/2027 and Assistant Expiring 9/2027. LPA also observed that the Licensees Pediatric First Aid/CPR is current Expiring 11/2027. LPA reminded (L1) that the Mandated Reporter Training must be renewed every two years. LPA Also observed that (L1) has filed children records as (L1) was reminded that own children also count and need to have a file for review from the department. LPA Alvarado and (L1) conducted a Health and Safety walk through of the facility indoors and outdoors. LPA observed that (L1) has a fully charged 3:A:40:BC fire extinguisher. During todays inspection LPA was able to clear three out of the four Deficiencies that were issued on 9/9/2025. Letters of deficiencies cleared were printed and provided to (L1). No deficiencies were cited during today's visit on 8/26/2025. See LIC 9102 Technical Assistance Regarding Operation of a Family Child Care Home. A notice of site visit was provided and must remain posted for 30 days. Appeal rights were provided and explained. Exit interview conducted and report was reviewed with Licensee, Gretel Avendano.
