Correction check·December 17, 2025

On 12/17/2025, at approximately 4:00PM, Licensing Program Analysts (LPA) Alvarado conducted an unannounced Plan of Correction (POC) visit at the facility. Present during the visit was (L1) supervising 4 children. (L1) was reminded that children who reside in the home are also part of the license during operating hours. The facility is a large license and is operating within capacity limits and ratio during today\342\200\231s inspection. All adults present have fingerprint clearance and are associated with the facility as of today\342\200\231s inspection. During the Annual inspection on 9/9/2025 LPA cited four deficiencies regarding Personnel Records and Physical plant. On 12/2/2025 LPA was able to clear three Plan of correction and due dates were discussed and agreed upon with (L1). Facility has one pending correction. During today\342\200\231s visit, LPA conducted Record review and was able to confirm that (L1\342\200\231s) Assistant has current required immunization's available for review. (L1) sent via text to LPA on 12/8/2025. During today\342\200\231s inspection LPA was able to clear the last remaining Deficiencies that were issued on 9/9/2025. Letters of deficiencies cleared were printed and provided to (L1). No deficiencies were cited during today's visit on 12/17/2025. A notice of site visit was provided and must remain posted for 30 days. Appeal rights were provided and explained. Exit interview conducted and report was reviewed with Licensee, Gretel Avendano.
