(b) There can only be one residence."This requirement is not met by evidence of: Based on interviews, observations, and record review, the facility did not comply with the section cited above as there is evidence to beleive that the licensee does not live in the home, which poses a potential health, safety or personal rights risk to persons in care.

CHILD'S RECORDS

102421(a): The licensee shall maintain, in each child's record, the signed and dated notice form required in Section 102419(d).This requirement is not met as evidenced by: Based on observation, interview, and record review, the licensee did not comply with the section cited above as there is a child present with no file and the rest of the files are incomplete which poses a potential health, safety or personal rights risk to persons in care.

OPERATION OF A FAMILY CHILD CARE HOME

102417(g)(4): Operation of a Family Child Care Home: Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children.This requirement is not met as evidenced by: Based on observations, the licensee did not comply with the section cited above, as LPAs observed detergents, medications and chemicals in Bedroom #1 & #2, and Laundry Room which were accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

102370(d): Criminal Record Clearance: All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility:This requirement is not met as evidenced by: Based on observation, interview and record review, the licensee did not comply with the section cited above as there is two adults (A1 and A2) living and/or working in the home that did not have criminal background clearance or associated, which poses an immediate health, safety or personal rights risk to persons in care.

INFANT SAFE SLEEP

102425(b):INFANT SAFE SLEEP: (b) Cribs or play yards shall be free from all loose articles and objects.This requirement is not met as evidence by : Based on observations, LPAs Tapia-Mandujano and Olgiun-Leon observed Bedroom #2 had play yards with mulltiple blankets inside and on the side of the play yards which poses a potential health, safety or personal rights risk to persons in care.

PERSONNEL REQUIREMENTS

102416(c): Personnel Records: The licensee and other personnel as specified shall complete training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866.This requirement is not met as evidenced by: Based on observation, interview, and record review, facility did not comply with the section cited above as Assistant (A1) was left alone with children and she does not have CPR & First Aid Certificate which poses an immediate health, safety or personal rights risk to persons in care.

102370(d): Criminal Record Clearance: All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility:This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as there was an unfigerprinted adult living and working in the home, which poses an immediate health, safety or personal rights risk to persons in care.
