What the inspector wrote

On June 17th, 2024 Licensing Program Analyst (LPA) Kassandra Medrano, conducted a subsequent site visit to the facility to deliver investigation findings. LPA met with Zhonglin Wang, Licensee, and Michael Shen, Facility Representative the purpose of the visit was explained. Michael was present and translated for Zhonglin as licensee speaks only Mandarin. Based on LPAs observations, record reviews, and interviews which were conducted. The allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Exit interview conducted and a copy of this report and appeal rights were reviewed and provided to licensee, Zhonglin Wang.
