What the inspector wrote

On January 22, 2026, at approximately 12:30 pm, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced Case Management visit. Purpose of visit was to hand deliver an amended LIC9099 report and obtain licensee signature on amended report. LPA met with Licensee Mojgan Ghasemzadeh and explained the purpose of the visit. Present in the home today was licensee and assistant caring for 6 children (4 infants & 2 preschoolers). All adults working in the facility have criminal background clearances and associated. Facility is currently operating within staff to child ratio on this date. LPA hand delivered the amended report dated January 6, 2026 and obtained a signed copy of amended report from licensee. Amended report and Notice of Site Visit was provided to Licensee, Mojgan Ghasemzadeh. Notice of Site Visit shall be posted for 30 consecutive days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted and report was reviewed with Licensee, Mojgan Ghasemzadeh.
