Correction check·July 29, 2025

On 7/29/2025 at 8:45AM., Licensing Program Analyst (LPA) Luis Gomez and met with Licensee, Lilia Zhambalova. The purpose of inspection was explained and was for an unannounced, plan of correction inspection established on 6/17/2025. Present was the licensee and 2 assistants caring for 10 children. (4 infant-age, 6 preschool-age) LPA inspected facility for health and safety hazards. During inspection, LPA conducted record review, observation, and interview. At 8:50AM., LPA observed functioning carbon monoxide/ smoke detector in facility. At 9:30AM., LPA confirmed missing child\342\200\231s files has been stored in the facility records. LPA reminded licensee to ensure siblings are each have their own, individual record. Licensee is maintaining napping logs for infant-age children in care. Napping logs reviewed included the children\342\200\231s name, and time/ date of review. Deficiencies issues during annual inspection were cleared today, and 'letter of deficiency citation cleared' was provided to licensee. An exit interview, and facility evaluation report was discussed with Licensee, Lilia Zhambalova. Licensee\342\200\231s signature of this form acknowledges the receipt of these documents. This report must be available in the facility for public review. Notice was provided and shall remain posted for 30 days.
