What the inspector wrote
On July 6, 2023 at approximately 8:30am, Licensing Program Analyst (LPA) Catrina Quimbo conducted a scheduled, pre-licensing inspection. LPA met with applicants, Francesca and Mary Lizel Francisco, and explained the purpose of the inspection.
Present during LPA's visit included both applicants, applicant's spouse/dad, and applicant's mother/grandmother. Co-applicants applied for a family child care home 06/21/2023. Applicants plan to operate Monday through Friday 7:30am to 5:30pm. Applicants plan to care for children ages 3 months to Kindergarten.
Applicants live in the home with their spouse/dad and mother/grandmother. Applicants and all adults completed livescan process on 07/05/2023. With applicants, LPA inspected the entire home for health and safety hazards. Facility is a single level home that includes 3 bedrooms, 1.5 bathrooms, living room, dining area, family room, kitchen, upper deck, lower deck and garage.
The
DAY CARE AREAS
are the living room, dining area, family room, bathroom #1 (next to family room), and upper deck. The
OFF LIMIT AREAS
are the kitchen, all bedrooms, bathroom #2, lower deck and garage. Applicants were notified any off limit areas are not to be used as day care areas without prior approval from department.
LPA observed the day care areas to be clean, equipped with a variety of age appropriate toys and materials. Flooring in day care area was observed to be clean. Applicants plan to utilize padded flooring for additional safety. LPA observed furniture to be age appropriate and in good working condition. Home is equipped with a fully charged fire extinguisher, fully stocked first aid kit, and a working dual carbon monoxide/smoke detector.
LPA observed electrical outlets in day care areas to have child safety covers. Per applicants, there are no weapons or firearms in the home. Bathroom was observed to be in proper working condition. Cabinet in bathroom was observed to have a child safety lock. Cleaning supplies, solutions and other hazardous materials will be stored behind child safety locked cabinets.

Entire backyard is fully fenced and enclosed with an at least 4 ft. high fence. Upper deck is to be utilized as a day care area while lower deck is to be off limits. Upper deck includes stairs that was observed to have a child safety gate. Railings in upper deck was observed to have additional child safety padding. Upper deck is equipped with a variety of outdoor toys and equipment that were in good working condition. LPA did not observe any pools, spas or bodies of water on the property.
Applicants plans to utilize play yards and mats for napping children. LPA advised applicants to use mats for children capable of climbing and walking. Applicants plan for children's families to provide sheets and blankets that will be sent home weekly or as needed for laundering purposes. Applicants also have sheets and blankets available for children if needed. Applicants discipline policy will be redirecting children. Applicants plan to isolate sick children in living room, separate from other children in care.
Applicants plan for children's families to provide food. Applicants also have a food service if needed. Food storage and labeling of children's materials were discussed. Applicants were reminded baby walkers, bouncers, jumpers and any other similar items are not to be used for children in care. There is a dog in the home that has their vaccination records made available for review.
Applicants have registered for CPR/First Aid and Preventive Health and Safety training that will be conducted 07/08/2023-07/09/2023. Applicants were reminded CPR/First Aid and Mandated Reporter training certification must be renewed every two years. All individuals whom work with children must have completed Mandated Reporter training certifications available for review.
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Applicants were reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicants and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed applicants of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
LPA reviewed with applicants the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted.
Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. During inspection, LPA was provided following from applicant:
-Pre-licensing Readiness Guide (LIC9217)
-Proof of control of property.
-Proof of MMR, Tdap, and Flu vaccinations for applicant, Mary Lizel Francisco..
-Proof MMR, Tdap and Flu vaccinations for applicant, Francesca Francisco.
Prior to recommended licensure, following must be completed:
-Completion of 16 hours of preventative health for both applicants.
-Completion of Mandated Reporter training for both applicants.
-Fingerprint clearance for all adults living in the home.
-Installation of child safety lock on kitchen cabinets (under sink and with medicine).
-Child safety door handles on off limit doors.
-Inaccessibility of fire place in living room.
