Inspector’s narrative

What the inspector wrote

On 11/3/2025, Licensing Program Analysts (LPAs) Wadhwa and Ly, arrived at the facility, unannounced to close the complaint investigation regarding the above allegation and met with Director Pam Cohn. Present at our inspection were 5 staff and 9 children. All staff present today were verified to have fingerprint clearance on file. During the investigation, LPAs conducted staff interviews, site observations and record reviews. Based on investigation and information gathered through interviews and evidence collected, the allegation 1.Staff use inappropriate discipline; the preponderance of evidence standards has been met; therefore, the above allegation is found to be SUBSTANTIATED. A Type “B” violation was issued today for the allegation that staff used inappropriate discipline in accordance with the California Code of Regulations, Title 22, Division 12, Chapter 1. See attached LIC9099D. A Plan of Correction (POC) was developed and reviewed with the Director. Exit interview was conducted and a copy of this report was reviewed and provided to the Director, Pam Cohn. NOTICE OF SITE VISIT WAS GIVEN AND SHALL REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
