What the inspector wrote

On February 29, 2024 at approximately 2:15pm, Licensing Program Analyst (LPA) Catrina Quimbo conducted a scheduled, pre-licensing inspection. LPA met with applicant, Stefanie Gindi, and explained the purpose of the visit. Present during LPA's visit included site supervisor and 7 school age children. Program is currently operating as a license exempt, "ELOP" program. Applicant plans to operate as a licensed program once license has been obtained. Application was received November 30, 2023. Fire clearance was received February 1, 2024. The applicant is requesting a school age license for 28 children (Tk to 5th grade). The facility will operate Monday through Friday for morning and afternoon hours. Morning hours will be 7:20am to 8:10am. Afternoon session will be 2:10pm to 6:00pm (Wednesdays 1:10pm to 6:00pm). The Champions after school program will operate at Allen Elementary School in Classroom C106 only. The designated site director will be Cynthia Sauceda. The facility was inspected, indoors and outdoors, for health and safety hazards. The school age program is exempt from requirements for indoor and outdoor square footage, fencing and toilet/sink ratios. No measurements were taken during LPA's visit. Indoors: The program will operate in classroom C106 only. Classroom is equipped with a variety of toys and materials that are in good condition. Furniture and equipment were observed to be new. Flooring in classroom includes carpets that were new and secured to the ground. Garbage bin in classroom was observed to have a tight fitting lid. LPA observed classroom to be equipped with a fully charged fire extinguisher, fire sprinklers, fire alarm system and a carbon monoxide detector. LPA tested carbon monoxide detector during visit, which was observed to be working. Classroom is also equipped with a fully stocked first aid kit. Applicant plans for medications with required authorization and licensing documentation will be stored in the classroom cabinets, in a locked box. Storage for children's belongings are located inside the classroom. LPA observed all toxins, poisons and chemicals to be stored in a locked cabinet, inaccessible to children. Isolation for a sick child will be in the classroom, separate from other enrolled children. There is a mat that will be stored in the classroom if needed for sick children. (Continue Report on Page 2...) (Continued, Page 2...) There is a sink in the classroom that applicant plans to use for hand washing only. Champions program will have access to two sets of bathrooms on site. Bathroom was observed to be clean and in proper working condition. LPA observed bathrooms to allow for individual privacy. Program will provide dry, afternoon snacks only. Food will be stored in a cabinet that does not contain toxins or poisons in the classroom. Water is accessible to enrolled children with a refillable water station on the school district site. Outdoors: The entire outdoor area of the school is enclosed by a fence that is at least four feet tall. Champions program has access to entire outdoor area that include multiple playgrounds, soccer field, basketball court and outdoor staging area. LPA observed playgrounds with play structures to have resilient padding underneath. Outdoor area is clean and free of any loose debris or materials. LPA did not observe any bodies of water on site. Applicant stated they are aware children are to always be supervised and monitored in the outdoor area. Applicant plans for children to go to each area in the outdoor area in groups. LPA reminded applicant children must always be supervised going to and from the outdoor area, the classroom and the restroom. Applicant stated they are aware disaster drills are conducted monthly and must be properly logged and documented. Discipline policy was discussed. Applicant plans to communicate and redirect children. Licensing documentation and posters were observed to be properly posted and made available for review inside the classroom. Children will be signed in and out of program with authorized representatives' written signatures. Applicant plans for sign in/out sheet to utilize electronic signatures in the future. LPA reminded applicant sign in/out records must be made available for review. This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed with applicant, the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. (Continue Report on Page 3...) (Continued, Page 3...) The requirement for Lead Water Testing was discussed (H&S Code 1597.16). Applicant was reminded a facility licensed after July 1, 2022, must have their water fixtures tested for lead 180 days after licensure and every 5 years thereafter. LPA advised applicant to review PIN 21-21-CCP. Per applicant, facility was built in 2022. Proof of building construction to be provided to LPA. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Based on fire clearance, maximum capacity facility can be licensed for is 28 children. Prior to recommended licensure, following must be completed: -Submission of proof of control of property. -Resubmission of documents indicated on Notice of Incomplete Application (LIC184C). -LPA's review and approval of documents resubmitted.
