What the inspector wrote

On June 11th, 2025, at approximately 12:00pm, Licensing Program Analyst (LPA) Tapia-Mandujano conducted a Case Management-Incident unannounced inspection. LPA explained the purpose of the inspection with Director, Jessica Smith. Facility is a single license that offers infant, toddler, and preschool age components. Present in the facility were Director, 8 infant children with 3 staff in Caterpillars, 14 toddlers with 4 staff between Ladybugs and Grasshoppers, and 30 preschool age children between Dragonflies and Honeybees. Owner, Navin Chugh arrived during inspection. All adults present during inspection have fingerprint clearance and are associated. Department was notified of an Unusual Incident Report on May 30th, 2025. Facility reported an incident that occurred on May 30 th , 2025 in the Infant classroom (Caterpillars) where a child had an allergic reaction and emergency services were contacted. During today's inspection, LPA interviewed Director and Owner, conducted a health and safety inspection, and obtained information about polices and procedures for administering medication. Based on interviews conducted, Child (C1) had an allergic reaction to egg in the Infant classroom. Per interviews conducted, parents for all children in the Infant classroom bring their own food and snacks. On 5/30/25, another child in the classroom brought eggs. Per interviews conducted, the child who brought eggs ate separately from C1 around 11:30am. Per interviews conducted, staff were aware of the egg that was brought on this day and separated C1 from child who was eating eggs. Staff cleaned off the feeding chair, cleaned the floor for any excess, and washed the child’s hand for C1’s safety. Per interviews conducted, it is not determined exactly how C1 could have been exposed to the eggs due to the staff taking safety precautions. At approximately 1:20pm, C1 started to display mild symptoms of an allergic reaction. Facility notified family of C1’s symptoms and due to the symptoms becoming more severe, staff had to administer epi-pen and contacted emergency services. C1 no longer needed medication and return to the facility on June 2 nd , 2025. Due to the incident occurring, Facility has asked all families in the program to no longer bring any egg/egg-based food items to the facility as of June 2 nd , 2025. Based on information and procedures being followed, department believes the facility followed appropriate procedures. No Deficiencies were cited today under CCR, Title 22, Div. 12, Chapt. 1. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with Director, Jessica Smith and Owner, Navin Chugh. A copy of this report was provided.
