101223: Peronal Rights: (a)(3): "The licensee shall ensure that each child is accorded the following personal rights...To be free from corporal or unusual punishment..."This requirement was not met as evidenced by: Based on record review of video footage there were three separate incidents observed of a child being handled in a rough manner. This poses an immediate health, safety and personal rights risk to children in care.

101216.3: Teacher-Child Ratio: (a) "There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance"This requirement was not met as evidenced by: Based on observation and record review of video footage, there were instances where the preschool classrooms were more than 12 children with one teacher present by stepping out of the classroom, which poses a potential health, safety or personal rights risk to persons in care.
