What the inspector wrote

On 11/12/2025 at 8:50 AM, Licensing Program Analysts (LPAs) Wadhwa and Gil conducted an unannounced, case management visit. LPAs met with the licensee Luciana De Oliveira and explained the purpose of the visit. Present during LPA's visit included the licensee, their assistant-spouse and 3 children (2 infants and 1 preschool age). The purpose of today\342\200\231s inspection is an increase of capacity. Licensee has requested to increase capacity from a small family child care home to a large family child care home. Capacity increase application was submitted to department on 8/4/2025 and fire clearance approval was obtained on 10/21/2025. Licensee lives in the home with their spouse. All adults living and/or working in the home have fingerprint clearance on file. Hours of operation are Monday through Friday 7:30 AM to 5:30 PM. The DAY CARE AREAS of the home include the living room, dining room, bedroom #1, bedroom #2, basement, bathroom #1, stairs for walking through only, and backyard. The OFF LIMIT AREAS are the kitchen, bedroom #3, bathroom# 2, and the garage. Off limit areas are made inaccessible with child safety door handles and child safety gates. Home is equipped with a fully charged fire extinguisher, first aid kit, multiple emergency backpacks and multiple smoke detectors. LPAs observed the carbon monoxide detector in the basement to not be working. Per Licensee, she was under the impression the smoke detectors were dual, smoke and carbon monoxide. LPAs confirmed the detectors were only smoke alarms and advised the licensee to have carbon monoxide detectors installed in each level of the home. Continued on Page 2.. Page 2 Capacity limits and ratios for a large family day care have been reviewed with the licensee on this date. LPAs reminded licensee that an assistant must be present when operating as a large license. LPAs reminded licensee when an assistant is not present, licensee must operate within capacity limits of a small family childcare home. Because the licensee rents/leases the home, proof of landlord notification is required. LPAs requested Licensee to provide a copy of the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. Licensee was informed she could also provide a copy of the signed Property Owner/Landlord Consent form (LIC 9149). LPAs will approve a large license, pending the submission of the following: 1. A signed copy of LIC 9151/ or signed copy of LIC 9149 2. A video evidence of a working carbon monoxide detector installed in both levels of the home. No deficiencies were issued during today's visit. A notice of site visit was given and must remain posted. Exit interview conducted and report was reviewed with the licensee, Luciana De Oliveira.
