What the inspector wrote

Centralized Application Bureau LPA Jamel Maiwandi met with Director of Learning Success Shelley Marquez and Site Director Laura Barraza for the purpose of an announced pre-licensing inspection. Applicant Ty Durekas was not present during this inspection. Applicant is requesting a school-age license to serve 72 school-age children ages TK and above. The facility is located at 340 Inverrness Drive, Pacifica CA, 94044. The facility will be utilizing Classroom B, classroom D, and the outdoor play area. The fire clearance was granted on 5/1/2024. The facility hours of operation are Monday through Friday from 7:00am to 6:00pm. The program offers before and after school care. Before care will operate Monday through Friday from 7:00am to 9:00am and after care will operate Monday through Friday from 1:15pm to 6:00pm. LPA reviewed with applicant the LIC 311A; records to be maintained at the facility, to include child\342\200\231s records, personnel records, and administrative records; and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. The facility will provide afternoon snacks, and children will bring their own lunch. INDOOR ACTIVITY SPACE: LPA inspected Classrooms B and D and the outdoor play area. LPA observed sufficient number of child size tables, chairs, and backpack racks. LPA observed a variety of age-appropriate toys and equipment. There is a first aid kit located in each classroom. Applicant stated medications and cleaning disinfectants will be stored in the food preparation area in locked cabinets and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. LPA observed a functional carbon monoxide detector in each classroom. Applicant stated they will be utilizing a digital sign in/out system and paper sign in/out system as a backup. Applicant stated water will be provide via filtered drinking water. Facility will be utilizing the office space in classroom B as the isolation area and use the staff restroom if necessary. Report continues on LIC 809 page 2 Page 2 Per Health and Safety Code1596.806 Exemption from square footage, toilet, isolation space, outdoor activity space, and fencing requirements, LPA did not measure the indoor and outdoor activity space. Per the Applicant, the facility will utilize the outdoor activity space and the school\342\200\231s restrooms. Facility submitted a certification statement on the lease agreement authorized by the school for use of the outdoor play area. OUTDOOR ACTIVITY SPACE: The facility will use the Sunset Ridge school campus outdoor play area. Play structures are approved for use by children ages 5-12. Shade will be provided by two large tarps during inclement weather. This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Applicant was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Licensee was encouraged to visit the Department website at WWW.CCLD.CA.GOV for child care updates, current forms, legislation and regulation information. A copy of this report will remain on file for a period of three years for public review upon request.
