Facility-requested visit·December 16, 2025

On 12/16/2025, at 9:00 AM, Licensing Program Analysts (LPAs) Wadhwa and Ly conducted an Unannounced Case Management Visit for an increase of capacity to this family child care home and met with Licensee Maria Luiza Pereira. Purpose of visit was explained. Present during the visit was licensee and an assistant (minor) caring for 5 children (4 preschool age children and 1 infant). At today's visit, LPAs and Licensee conducted a health and safety inspection of the home. LPAs observed the home is clean, orderly and properly ventilated. Telephone service, heating, lighting and ventilation were evaluated. LPAs observed a fully serviced 2A10BC Fire extinguisher, and a working smoke and carbon monoxide detector. Day care areas of the home are on the ground level of this multi-level house. The areas include the living room \342\200\234Play Room\342\200\235, the kitchen, bathroom and a bedroom \342\200\234Nap room\342\200\235 and the backyard. Entrance to the day care is through the backyard and the second exit is through the nap room. Fire inspection was conducted on 12/5/2025. LPA Wadhwa received a telephone confirmation of fire clearance from Fire Inspector on 12/5/2025. On this day, Licensee also submitted an updated copy of LIC279 and LIC 279B to include her adult son\342\200\231s information on the application. Prior to approval of a large family child care home, the following must be completed: 1. A copy of the fire clearance STD 850. 2. Licensee to submit a copy of LIC 9149 and LIC 9151. No deficiencies were issued today under Title 22 Division 12 of the California Code of Regulations. A copy of this report and appeal rights were discussed and left with Licensee whose signature on this form confirm receipt of reports . Notice of Site Visit was provided to licensee.
